Please fill out the first page including the project address, name of contractor and or owner, and a contact phone number where you can be reached. Include a detailed description of the project, plans may be required. The second page, the license agreement, must be turned in with a signature, no photo copies please. The first page may be submitted online, faxed to the City of Bishop Public Works Department at 760-873-4873 or emailed to publicworks@ca-bishop.us. Upon review of your application, you will be contacted with the permit fee. If you have any questions, please call 760-873-8458.
